The complicated and combined course of alimentary toxicoinfections are practically the main causes of all the lethal outcomes in these diseases. In this work the data on the recent progress in the rehydration therapy of acute enteric infections are presented. The authors established that combined forms constituted 15.3% and complicated ones constituted 2% of alimentary toxicoinfections. The results of the treatment of alimentary toxicoinfections combined with ischemic heart disease, hypertension and chronic alcoholism, as well as complicated by shock and pneumonia, are presented. The general principles of treatment in combined and complicated cases are the maintenance of the usual (under given conditions) volume of liquid for rehydration with the limitation of the rate of its introduction.

The study has shown that the fibronectin-binding properties of different Salmonella species can be tested by means of passive hemagglutination with fibronectin-sensitized formulated sheep red blood cells (SRBC). S. enteritidis and S. typhimurium isolated from foodstuffs, as well as the representatives of the species S. kottbus, S. anatum, S. tshiongwe and S. newlands, are capable of binding fibronectin. Fibronectin-binding properties have also been detected in Shigella sonnei, as well as in eubiotic microorganisms Escherichia coli M 17 and Lactobacillus plantarum. In pH ranging from 6.5 to 7.4 salmonellae, shigellae and E. coli agglutinate fibronectin-sensitized SRBC much more effectively in acidic pH values, while changes in pH have been found to produce no essential influence on the fibronectin-binding capacity of Staphylococcus aureus and lactobacilli.

Therapeutic effectiveness of an oral rehydration solution of the second generation Super-ORS based on rice and salt has been tried in 28 patients with food poisoning. Administration of the solution produced a marked clinical effect superior to that of the first generation solutions Glucosalan, Citroglucosalan, Rehydron.

The data are presented on an outbreak of salmonellosis induced by S. enteritidis. Gastrointestinal symptoms were recorded in 97.8% of 90 patients, 2.2% of them developed a generalized form. The disease ran a moderate course in 84.5% and a severe course in 2.2% of the patients majority of whom suffered from intoxication and dehydration. The therapeutic response was achieved upon rehydration treatment with crystalloid solutions.

In rabbits, Shigella endotoxin causes hypoxaemia, metabolic acidosis, and hypocapnia. These changes are more marked at a dose of 1 mg kg-1 than with one of 0.5 mg kg-1 body weight. The endotoxin also reduces the plasma level and heparin-binding activity of fibronectin (FN). During convalescence, FN activity recovers at a slower rate than its level. Hyperbaric oxygenation (HBO; 2 atm for 1 h) normalizes blood gas composition and acid-base and electrolyte balance, improves the clinical picture of endotoxaemia, enhances FN activity and, to a lesser degree, its concentration, and lowers mortality. Indomethacin (IN) (5 mg kg-1 orally) has a similar action. When HBO and IN are used in combination, there is no enhancement of the therapeutic effect and mortality increases.

The authors suggest a method for the detection of S. aureus and S. epidermidis strains fibronectin-binding ability (FNBA), based on a new principle--the passive coagglutination test. Using this test, FNBA was detected at 16 of the 20 tested S. aureus strains and in 6 of the 11 S. epidermidis strains. This method yields more demonstrable results than the flake-formation test on the glass in the presence of fibronectin. A nonagglutinating fibronectin form may be used in the test.

Blood plasma content of fibronectin was studied in 125 patients with food poisoning using rocket immunoelectrophoresis. enzyme immunoassay and antibody neutralization reaction. During the development of the process the content of fibronectin was reduced in most of the patients. This parameter usually drops at the peak of the disease and is normalized on recovery. Measurement of fibronectin in blood plasma can be an additional criterion for determining the degree of severity and the course of the disease and recovery of patients. This method that makes use of the antibody neutralization reaction is simple, can be rapidly performed and produces accurate results.

A study was made of the influence of the level and biological activity of plasma fibronectin of phagocytosis and opsonic activity of blood plasma in 147 patients with food toxinfections depending on a period and degree of severity of a course of disease. Plasma fibronectin concentration was determined by rocket immunoelectrophoresis. Fibronectin biological activity was assessed by cold heparin precipitation. Opsonic activity of blood plasma before and after fibronectin inactivation in it was investigated using indices of phagocytosis which was studied by optical microscopy. Food toxinfections were attended by a decrease in the level and biological activity of plasma fibronectin returning to normal during therapy and convalescence. A degree of the reduction of the level of plasma fibronectin reflected the severity of a course of food toxinfections. Phagocytic insufficiency developed in food toxinfections. One of its mechanisms was a decrease in opsonic activity of plasma fibronectin as a result of the reduction of its level.

Altogether 320 elderly and old-aged patients with alimentary toxinfections suffering from essential hypertension (65.9%), coronary heart disease (39.3%), atherosclerosis (29.3%) were observed. A severe course of toxinfection was noted in 5.9% of the patients, an average severe course in 94.1%. The patients received intravenously the "quartasol" solution in full volume (25.7%) or the ORS solution with sodium hydroxycitrate per os (74.3%). Therapeutic efficacy was high. Antihypertensive therapy and antianginal drugs were simultaneously administered. Neither complications nor side effects were observed during therapy of the elderly and old-aged patients with alimentary toxinfections. In addition to the above therapy all the patients received sparing diet and enzyme preparations.
